HELLO GRANTEES!

To get things ready on our end to disburse your grant payments (in December 2021 and July 2022), we need to gather some information about your organization and, if applicable, your fiscal sponsor.

Grant payments will be sent via paper check, or we can send it electronically directly to your bank account. We use the bill paying platform Bill.com to process payments.

Please complete the Grantee Information Form below, which will also prompt you to upload a signed W-9 Form of the entity we will be remitting payment to (you or your fiscal sponsor). You can download a blank, fillable W-9 here.
